Inspired by the Business Week customer service report.
In rating please think about two things:
How customer friendly are the companies policies (returns, reservations, etc) and how helpful (knowledgeable or friendly) are the people.

This is really idiotic - most people will only have had the opportunity to talk / deal with one or two various customer services throughout their lives. This survey practically expects that everybody has had an opportunity with each at least once. It's uncompromised results depend on the probability that everybody has experienced each scenario at least once and so the survey, by pure nature, will produce compromised results.
